Accident Investigation

All accidents and Incidents reported to the OSH Agency are investigated in accordance with the OSH Act (section 46 of the OSH Act). Accidents are to be reported on the prescribed form. Incidents are to be reported in writing to the Chief Inspector. Reporting requirements are outline in the notification tab.

The OSH agency classifies accidents in three categories:

  • Fatal,
  • Critical  
  • Non-critical

The purpose of the investigation is to determine;

1 The causes
2 Whether action has been taken or needs to be taken to prevent recurrences
3 If there was compliance with the law
4 Lessons to be learnt that can influence the law and guidance material
5 The appropriate action to be taken for breaches of the law
